Informed Consent Template

The Providence Health Care consent form template, which is modeled on the UBC Clinical Research Ethics Board sample consent form, is provided to assist coordinators and investigators in producing consent forms which meet UBC PHC Research Ethics Board requirements.

This consent form template contains references to the required informed consent elements as described in section 4.8.10 of the ICH-GCP Guideline. These are included for reference only.

Please ensure that you review the Providence Health Care Research Site each time you prepare a consent form, as notices of updates in the UBC or PHC Guidance Notes may be added from time to time.

ICH GCP Consent Form Elements Check List

All consent forms used for clinical trials must include elements of consent as described in the ICH Guidelines (section 4.8.10). It is highly recommended that all other consent forms include the same elements, appropriate to the type of research being conducted. The REB uses this checklist when screening applications for ethical review.
