Post Transplant Clinic

The Post-Transplant Clinic supports patients and their families for the entire life of their new kidney. Immediately after discharge from hospital, our team will see patients up to twice a week, a schedule that will eventually decrease to once every 3 or 4 months.

Post transplant services include: follow-up care and education for all renal transplant patients, monitoring patients by lab work, clinic visits and telephone, and facilitating transfers to and from regional transplant clinics and other transplant centres.
