Education, clinical care, and information for patients and their families with chronic kidney disease. Care is provided to patients at all stages of kidney disease prior to the patient receiving dialysis or a kidney transplant or throughout conservative treatment
The overall goals for the service include:
- Early detection, prevention and minimizing complications and delaying progression of kidney disease.
- Ensuring patient and family understanding of kidney disease.
- Ensuring understanding of the health care system, hospital and outpatient systems and services available to kidney patients.
- Identifying potential issues related to long-term patient management.
- Facilitating longitudinal care of patients with Chronic Kidney Disease.
- Integrating specialist care using a coordinated approach - Integrated/ Shared Care Clinic.
Proximity of the clinic to other services of the Renal program such as the hemodialysis, peritoneal dialysis, and inpatient nephrology units assists in coordination, integration, and delivery of care to chronically-ill patients.
